1) Certified Birth Certificate,
2) Social Security Card,
3) Original Gold School Physical Form (both sides completed),
4) Original Blue Florida Immunization Record
5) Proof of Residential Address (Utility Bill in parent’s name with physical address).
Parents need to call the school at 762-3121 to schedule an appointment.

Terence L. McCreedy, 77, of St. Petersburg, died Thursday, April 25, 2013, at Westminster Suncoast. Born in South Milwaukee, he moved to Florida in 1998 from Buffalo, N.Y. He graduated from South Milwaukee High School Class of 1953. In 1960 he graduated Cum Laude from Lakeland College in Sheboygan, WI with a Bachelors of Arts degree and received his Bachelors of Divinity from United Theological Seminary of The Twin Cities in 1963. He was the minister at Grace Parkridge United Church of Christ in Buffalo, N.Y. He was a member of First Baptist Church of St. Petersburg. He was a devoted father and grandfather who loved spending time with his family. He was preceded in death by his wife Margaret, father Lester and mother Helen. He is survived by his three sons, Scott M. (and Kathryn) McCreedy of St. Petersburg, Fla.; Keith A. (and Debora) McCreedy of W. Seneca, N.Y.; and Jonathan D. (and Carrie) McCreedy of St. Petersburg; his sister, Nadine, of Milwaukee; and his six grandchildren, Danielle, Ryan, Will, Tess, Eavan and Micah.

Anna S. Moon, 80, passed away on Thursday, April 25, 2013. She was born on February 27, 1933 in Wessling, Germany, to Heinrich and Anna Steininger. She is survived by her daughter, Sandra Wiley of Dallas, TX; son, John Moon of Satellite Beach; and sister, Bertha Sheidl, (her son, Peter), of Germany. A gathering will be held on Tuesday, April 30, 2013 from 12-1 pm with a service to begin at 1 pm at Beach Funeral Homes – East, 1689 S. Patrick Dr. Indian Harbour Beach. Anna was predeceased by her husband, Harold, in 2011. Both Anna and Harold with be interred at Arlington National Cemetery at a later date. In addition to flowers, donation may be made to Holy Name of Jesus Catholic Church, Indialantic.
McDougal, Ernie Nathaniel “Papa Ernie” 72, of Pinellas Park, died Saturday, April 13, 2013, surrounded by his wife and family at Bayfront Hospice. Born in Haywood, OK, he came here in 1979 from Hagerstown, MD where he worked for Mack Trucks. Locally, he retired from Southern Tool and Machine Company in 2006. Ernie was an Air Force veteran with a passion for classic Mustangs, movies, building model airplanes, scuba diving and his dogs. He was preceded in death by his parents, Henry and Byrtie McDougal. He is survived by his loving wife, Gail; sons, Richard (Kimberly), David and Dennis; 4 grandchildren, Zachary, Jacob, Lexie and Sean and mother-in-law, Fran Fox. Relatives and friends are invited to gather for a memorial at 11am on Wednesday, June 19th at Anderson McQueen, 7820 38th Ave. N. Private interment to follow at Bay Pines National Cemetery. In lieu of flowers, donations may be made in his name to the SPCA of Tampa Bay or the Wounded Warriors Foundation.

Mrs. Johnson received the ultimate healing from Jesus Our Lord and Savior on April 27, 2013 at Kindred Hospital. She was born Margaret Ruth Brown on December 29, 1926 to Mary and Charles Brown in Corydon, Indiana. She married Noble M. Johnson on July 13, 1946. She and Noble retired to their beloved La Feria, Texas in 1978 where she lived until 2012. She was a member of the American Legion Auxiliary Unit 14 in St. Petersburg, Florida and attended First Baptist Church of St. Petersburg. She was predeceased by her husband of 57 years, Noble. Survivors include daughters, Terrie Pickett (Wayne) St. Petersburg, Florida, Myra Smith (Gary) Georgetown, Indiana, and two grandchildren, Kayla Lee (Daniel) New Albany, Indiana and Gary M. Smith, Charlestown, Indiana. Contributions may be made in her memory to First Baptist Church of St. Petersburg and Hospice of the Florida Suncoast. A private family memorial will be held at a later date with burial at Heavenly Grace Memorial Park in La Feria, Texas
